Lightning cup is only in Mario Kart: Super circuit for GBA so it's not here sorry.

Play online. If you've beaten Mirror All Cup Tour and everything before it, then no.

Well, you can look for things out of the ordinary like shortcuts, scenery, etc. And you say you've beaten the game, right? Then you would have found out about a Lightning cup by now!
