Can anybody name all the 21 or so packs? So far I have 17,

-Mystical Elf
-Red Eyes Black Dragon
-Harpie Lady
-Kazejin, Suijin, Sanga of the Thunder
-Great Moth (or Perfectly Ultimate Great Moth, I can't tell)
-Blue Eyes White Dragon
-Relinquished
-Buster Blader
-Blue Eyes Ultimate Dragon
-Black Luster Soldier
-Judge Man
-Tiger Axe
-Millenium Puzzle Blue
-Millenium Puzzle Green
-Millenium Puzzle Gold
-And another pack, I don't know the monsters name, But it looks like a lizard.
Can anybody tell me what packs I'm missing and how to get them?
