The Item Up-Grade is used to Evolve porygon to Porygon-2. All you do is give Porygon the item and then Trade it with a friend.

You give it to a porygon and Trade it to either Pokemon Ruby/Sapphire/Emerald and so on and it should Evolve into porygon 2
