Not you don't need/have to collapse and go to the hospital and cliff will be kicked out of the bed and avoid dying, just give him a job on duke's place..

The only thing that happens if you faint while Cliff is in the hopsital bed is that a hilarious clip is shown of Doctor kicking him out of the bed. Cliff falls on the floor and then Ellie drags his unconscious body off-screen.
When you're offered to help at the winery by Duke, take the job and invite Cliff. He'll leave town forever if you don't take the job and/or don't invite him.
