To continue your game you must have the Hudson Bee which is somewhere in World 1. When your game is over hold left and press start! You can select which island you want to start on by pressing the following combination just after the coconut hits the man on the head at the title screen: Right, Left, Right, Left, A, B, A and B. Then simply press up or down to select your starting island and press A to start your game.
