After a while of feeding it and playing with it 1 month max it will evolve. Stay in your chao garden as much as possible to get your evolved chao quicker.
The more chaos emeralds he has, the more powerful he becomes. To see how many chaos emeralds he has.
To see how many he has, you can count them or just check out the number at the end of his name.
If you're talking about the little blue things, and by 'form' you mean start getting special features - they get them when you give them animals (that you get from levels).
