05. Hitman: Contracts -- The Ghost in the Mirror
The Hitman series is one of the longest-running and most successful stealth-action assassin games ever to exist, which means that it pretty much spans the video game tech generation layers, with the early games being kind of strange and crude looking compared to the more recent ones, but hey at the time that was totally cutting edge man!
In Hitman: Contracts, during the mission Traditions of the Trade, Agent 47 is dispatched to the Thermal Bath Hotel, where he has been sent to do what he does best - dress up in a dark suit with a red tie and do the tango! Kidding! Just kidding.
Actually what Agent 47 needs to do is assassinate a terrorist and recover a chemical bomb - but in the process of doing that, assuming you are working towards the Assassin Rating, you will find yourself in a closed off section of the hotel where you encounter crime scene tape and cops. And a murder scene!
If you enter each of the rooms in that hallway and go into their bathrooms, when you look in the mirror you will see the ghost of the slain guest!
Working your way from room to room, eventually you will arrive at the scene of the crime - the bathtub full of blood and the knife stuck in the wall is a clear hint.
Bonus SEE - Kill the Ghost! -- Yes, you read that right, as a bonus if you are paying close attention - and have lightning quick reflexes, when you leave the room to go back in the hall the ghost appears for the briefest of moments and, if you are fast enough with your garrote you can kill it!
Seriously - you can actually kill the ghost with Agent 47's signature weapon... Not only that but you also unlock a special Trophy / Achievement called “Ghost Buster” as a bonus-bonus! Now how cool is that?!

for gta 5 you can actually go up closer when she disapeers and aim in and you still can see her so that video is jank.

I saw the headless horse rider in sky rim once but was on foot and couldn't actually keep up with them, it was creepy but nothing actually happened to me when seeing him xx and i'm sorry I can't remember where I was :/ it wa a snowy road that's all I know xx[b][/b]
