In the opening area of the Latveria mission, there is a Courtyard not too far before Electro that features a statue of Dr. Doom. Simply destroy the statue and the achievement is yours.

To get the high scores, you need to fill up the meter that appears in the bottom right corner of the screen. If you complete it, you will get a healing token as a reward. The high score depends of the type of fusion.
Targeted: Only 1 enemy is required to get the high score with these. Save them for bosses or the larger robots.
Clearing: These usually require about 10 enemies to get the high score. Save them for large amounts of enemies that are all close by.
Guided: These usually require 12 enemies for a high score. Save them for large amounts of enemies that are more spread out around you.
