Below are a list of the medals available in Resident Evil: The Mercenaries 3D, unlocked for getting the specified ranks on each level:

Newcomer: Reach rank B or higher on levels 1-1, 1-2 and 1-3.

Beginner: Reach rank B or higher on levels 2-1, 2-2 and 2-3.

Rookie: Reach rank B or higher on levels 3-1, 3-2, 3-3, 3-4 and 3-5.

Veteran: Reach rank B or higher on levels 4-1, 4-2, 4-3, 4-4 and 4-5.

Captain: Reach rank B or higher on levels 5-2, 5-2, 5-3. 5-4 and 5-5.

Hero: Reach rank B or higher on levels Ex-1, Ex-2, Ex-3, Ex-4, Ex-5, Ex-6, Ex-7, Ex-8 and Ex-9.

STARS: Reach rank A on all missions.
