I went back into a world after I completed it how do I get out please

Go to where you would save your game (the blue-green circle on the floor) then examine it then it will give you 3 choices (save game, world map, and drop) if you just want 2 get out then choose world map, if you want to just change from sora to riku or riku to sora choose drop

Find a save spot without any enemy dream eaters around, hit the "A" button, scroll down to world map/leave world, hit the "A" button.
