If you still need help with this, what you need to do is get an axe from upstairs. They are hanging on the wall in glass cases and can be obtained by breaking them. Once you have one, either execute a hunter or stand above a hunter's corpse and hit square. Then you take a head and equip it then walk up to the door and hit circle.

To get past pickman, what you need to do is kill the orderly coming down the stairs and take his tranquilizer gun then shoot pickman with the tranquilizer gun.

You should have a tranq. Gun, so use it on him so He'll fall unconscious, and you can continue on with the game.

Lol, I had problems with this myself. There are 3 speakers you have to shoot, and they should be on your map thing. The first one is in the room, after you bust the mirror in the top right corner I think. It's fairly easy to shoot.

The second is the room down the halls. You should be facing a pole barrier, and if you step to the right and look past it, you should see the speaker up on the wall. In this room, is also a guy, and when you try to lock on to the speaker, you'll turn around and lock on to the guy instead. So you need to free aim, aim your gun, and press the right analog stick, and you should see the targeter change.

Then shoot the speaker, and give a head shot to the guy. The third speaker is on the wall in the halls, I can't remember exactly where but It's fairly easy to get. I remember it's aboves a shelf. After that you'll have shot all the speakers, and got rid of that Infernal Racket. You should now be able to continue through the level.
